Projects in the Applied Mathematical Sciences
You can use this form to apply for Math 203 in either of two ways:
1) Cut and paste this page into an e-mail program (either download it
first, or take it directly from your browser), fill it out, and
e-mail it, either as an attachment or as text in an email, to
Professor Slobodan Simic (simic@math.sjsu.edu). This option is
strongly preferred.
2) Alternately, print this page, fill it out by hand (type or print neatly), and either:
a) submit it to Professor Slobodan Simic at the Math Office,
308 MacQuarrie; or
b) send it by campus mail to Professor Slobodan Simic, Math
Department, SJSU, extended zip 0103.
Please do not put printed applications under Prof. Simic's door or in his office, as
it is more likely to get lost that way.
